Community access
Community access is also interpreted as social and community participation in participant’s NDIS plan. Hope Circle explores participant’s individual and unique interest and links them to the right support workers who have relevant experiences or interest to support participants attending social and community events. Community access support includes a range of services to assist participants with grocery shopping, attending medical appointments, meaningful and entertaining activities such as visiting an art gallery, zoo, fishing, walking in the park or along the beach.
In this service, transport support can be challenging for some of the participants or tricky due to lack of transport funding in the plan. Hope Circle is willing to work with participants in exploring a strategy or solution to overcome this barrier and maximizing participant’s opportunity to connect and engage with the community.
Participants can utilize the core support funding in their plan to access this service.
