Community access
Community access is also interpreted as social and community participation in the participant’s NDIS plan. Hope Circle explores participants’ unique interests and links them to the right support workers with relevant experiences or interests to support participants attending social and community events. Community access support includes services to assist participants with grocery shopping, attending medical appointments, and meaningful and entertaining activities such as visiting an art gallery or zoo, fishing, and walking in the park or along the beach.
In this service, transport support can be challenging for some of the participants for various reasons, including limited funding in their NDIS plan. Hope Circle works alongside our participants and their care team to explore a strategy or solution to overcome these barriers while maximizing the participant’s opportunity to connect and engage in the community.
Participants can use the core support funding to access this service.
